Please Note: We implemented our online only application system in 2020, and the "grace period" for paper applications ended December 31, 2020. We are dedicated in helping to find solutions for athletes, caregivers, and coaches who have any technological difficulties with our athlete application. We sincerely appreciate your cooperation in helping us move to more efficient technological improvements in order for us to dedicate more staff resources to serving our athletes. We will do everything we can to find solutions to help everyone fill out our online athlete application.

Please help us out by throwing away, or deleting any blank paper copies you have. Any blank paper applications that are in circulation are currently outdated. SOCO is no longer distributing paper copies of our athlete application. Please reach out to our Help Desk for help with filling out our online application.

The exam form must clear the athlete to participate in Special Olympics and/or sports. If the athlete is a person with Down Syndrome, the exam form will need to clear the athlete for ATLANTOAXIAL INSTABILITY (AAI).

Special Olympics Colorado can provide assistance "over the phone" or "in person" by appointment to help you fill out the online athlete application. With the exception of National Holidays, our Athlete Application Help Desk hours are generally from 10:00am-12:00 noon, Monday-Friday.
