Please ensure you have registered for all of your sports, HERE. All athletes, and Unified partners will need to register for each sport they are participating in, each season.

The Athlete Application is required for participation as a Special Olympics Colorado athlete! Athletes should complete the online application below, which also requires you to upload a Physical Examination. Use the resources below to help you complete the application!

Have a licensed Medical Professional qualified to conduct exams & prescribe medications fill out the Physical Examination Form (form in Spanish) or a Sports Physical that clears you for athletic participation.

Upload the completed Physical Examination or Sports Physical Form here and continue filling out the athlete application.

If your doctor filled it out online, please use the “Save and Continue” link you received via email to continue your application.
